Choices start before the final dialogue option. The quests you finish, people you ignore, time you spend and Coen's hunger can all change a scene. Official demos show Gremla and Esme suffering different outcomes when Coen spends his limited time elsewhere, while a depleted hunger meter can replace every dialogue option with an unavoidable demand to feed.

What can change an outcome

Gremla's pig shows how a small choice branches
In the official demo, returning Gremla's pig reveals that the villager plans to slaughter it. Coen can join in, refuse or buy the pig if he has enough money. The useful question is not only which dialogue sounds kindest, but what resources and later outcome the choice creates.
Skipping help can decide a later scene
The demo also gives Coen objectives involving healing herbs for his mother and help for Gremla. Ignoring those needs changes who survives the later Blood Mass. The game continues after those deaths, carrying the result into the rest of the run.
Hunger can remove the choice entirely
When the blood hunger meter empties, Coen can lose the normal conversation options and be forced toward feeding. Check that state before a meeting where you care about the outcome.
